STUDENT APPLICATIONS

Please follow the steps below to enroll in one of our after-school art clubs:

1. Find your school & club below. Click on the link.

2. Print out the application & waiver.

3. Students need to complete the Application portion (front & back). A parent/legal guardian needs to fill out & sign the Waiver of Liability.

4. Return completed Application & Waiver to your school’s point-of-contact listed at the top of the Application… or scan and email to hello@lorienacademy.org.
